Abstract

Caseous calcification of the mitral valve annulus is a rare variant of mitral annular calcification making up about 0.6% of cases. The diagnosis of CCMA has previously been challenging but has come a long way with further advancements in cardiac multimodal imaging. The mechanism behind the progression of MAC to CCMA is poorly understood however has been described more commonly in patients with advanced age, female gender, CKD, and abnormal calcium phosphate metabolism. Herein, we present a case of a female patient who presented with multiple ischemic strokes who was found to have a massive caseous mitral annular mass in the setting of ESRD and functional parathyroid adenoma.
