Abstract

A Nursing Professional Development (NPD) Specialist identified 90% of first-year turnover occurred with experienced nurses in the acute care setting, which accrues a cost of $60,000 per nurse. Experienced nurses departed within the first year of hire due to the work environment and personal reasons. This high-risk population had minimal targeted training, specifically nurses with greater than six months of experience. A survey of newly hired experienced nurses revealed interest in a program to support their transition. An experienced nurse fellowship program was created to promote a healthy work environment, professional growth, and retention by allowing organizational assimilation. The program was developed based on current literature, experienced nurse survey feedback, and accreditation standards. It is offered to newly hired nurses within 60 days of hire, with recruitment focused on nurses transitioning into an acute care setting from outpatient, long-term care, and international settings. Guided by NPD Standard 13, Education, and utilizing adult learning principles, the program includes innovative virtual simulations, peer socialization, networking, and reflective practices. Additional topics include role development, health equity, and mentorship. Program metrics yielded a successful return on investment, with 100% retention and favorable participant evaluations. Fellows reported feelings of connectedness, companionship, peer support, and organizational pride. Additionally, 95% of the fellows felt the program provided a successful organizational transition. Fellows requested more networking time, so a virtual café was created to foster further enculturation, career advancement, and overall well-being post-program. Based on the initial findings, plans for further expansion and accreditation are underway.
