Nurse-driven Innovations in Care Redesign and Delivery poster presentation at Elevating Nursing Excellence: Purpose, Profession, Passion; Advocate Health Midwest Region Nursing Research & Professional Development Conference 2024; November 13, 2024; virtual.

Abstract

Introduction and Context: Advance Care Planning (ACP) activities support the incorporation of patient values into medical decision making and have been shown to positively impact patient’s perception of care and reduce the likelihood of utilizations. Despite strong evidence in support of ACP, there is an underutilization of ACP services in ambulatory care. Ambulatory Care Management (ACM) programs engage individuals with chronic conditions in self-management strategies to contribute to reducing preventable utilizations. The aim of this project was to develop and implement an ACP program to support clinical teammates in ACP, with the goal of increasing engagement in ACP activities amongst individuals living with chronic conditions.

Implementation Strategy: Clinical teammates, Registered Nurses and Social Workers, in ACM were selected as priority learners to engage in Respecting Choices Next Step Advance Care Planning facilitator certification. Following training, facilitators incorporated ACP into their daily practice, supported through coaching and peer mentorship. Conversation data and facilitator feedback were used to identify areas of opportunity, and clinical teammates engaged in process improvement initiatives.

Outcomes and Impact: Between January 2023 and May 2024, 48 ACM Teammates completed 1399 ACP conversations and facilitated 511 ACP documents. Facilitators engaged in 15 group mentorship sessions and over 75 individual coaching sessions: sharing case studies, clinical development strategies, and learning from peers and experts. Facilitators established working partnerships with Health Information, Clinical Informatics, and Advance Care Planning Shared Decision Making in Serious Illness (ACP/SDMSI).

Insights: Training and ongoing mentorship led to an increase in ACP volumes each quarter. Mentorship model and collaborative partnership provided facilitator feedback to develop EHR tools, data abstraction methods, educational resources, and communication processes.

Implications: Model of care for ACM clinical roles provides time, opportunity, and an environment to engage individuals in ACP activities. With structured training, data collection processes, and feedback, Registered Nurses and Social Workers effectively increased patient engagement in ACP activities.
