Abstract
Introduction and Context:
Workplace Violence (WPV) within healthcare settings poses a significant challenge, leading to substantial organizational costs stemming from teammate stress and injuries. A nurse-led interdisciplinary team undertook a proactive approach encompassing early identification of escalating behaviors, enhanced accessibility to de-escalation resources, and improved communication strategies of identified violent patients. These interventions sought to effectively mitigate WPV incidents with teammate injuries.
Implementation Strategy:
A multi-modal pilot implemented on four adult inpatient units at a large teaching hospital. The Brøset Violence Checklist (BVC), a validated tool used to score a patient’s potential for imminent violent behavior, was adopted from behavioral health to inpatient units. Patient Intervention Bags (PIB) were developed with at-the-ready resources. Additional identification tools were implemented including specific icons in the patient chart, and indicators on meal tray tickets and handover communications. The pilot’s success paved the way for adoption of these innovative strategies and establishment of site-specific guidelines. Dissemination of these best practices occurred through various channels such as shared governance forums, town hall meetings, leadership assemblies, and internal communication platforms.
Outcomes and Impact:
In 2023, incidents of Workplace Violence (WPV) resulting in teammate injuries exhibited a substantial decline from 68 documented cases to 15, indicating a 78% reduction in teammate injuries when juxtaposed with pre-project data.
Insights:
Teammates easily integrated Brøset into their workflow. They valued the PIB and appreciated enhanced communication. These interventions are a cost-effective strategy in contrast with the financial repercussions of teammate injuries, averaging $39,509 per occurrence, and the annual healthcare costs attributed to workplace stress per teammate, estimated at $358. This cost-benefit analysis underscores a favorable return on investment.
Implications
These innovations emphasize the importance of patient and teammate safety and well-being. Embracing a culture of innovation, compassion, and excellence, this approach signifies a step towards exemplary healthcare delivery. The interventions are adaptable for seamless integration across broader healthcare settings.
