Quality Improvement poster presented at Transforming Practice: The Intersection of Technology and Nursing Excellence; Advocate Health Nursing Research and Professional Development Conference 2025; November 12, 2025; Virtual.
Abstract
Background
Patient medication adherence is a critical determinant of patient outcomes, cost reduction, and performance metrics. Quality improvement opportunities around medication adherence were identified within Ambulatory Care Management, including a lack of tools, resources, and foundational knowledge.
Purpose
The primary aim of this interdisciplinary initiative was to identify gaps in current practices, understand the challenges surrounding medication adherence, and develop evidence-based strategies equipping nurses with the necessary tools and knowledge to address patient-specific barriers.
Implementation Plan
A comprehensive literature review was conducted identifying best practices for improving medication adherence. A survey helped identify knowledge gaps, barriers, and nursing confidence in addressing adherence. Using the survey results and literature review, a Resource Guide was created, categorizing adherence solutions by medication types and barriers. Additionally, a webinar promoted engagement and understanding on the nursing impact and intervention in medication adherence. A Medication Adherence Assessment Tool was integrated into Epic to systematically identify and address adherence risks. A post-survey is planned to measure intervention effectiveness.
Outcomes
The initiative saw 100% engagement from Care Managers (n=112) in the Continuing Education webinar focused on Medication Adherence in Chronic Condition Management. The Medication Adherence Resource Guide was utilized by 88% of Care Managers in practice. The integration of the Medication Adherence Assessment Flowsheet into assessment documentation facilitated the identification of adherence barriers. These structured resources and tools optimized the nursing role in improving adherence in chronic condition management.
Implications for Practice
This initiative demonstrates the value of structured, interdisciplinary, evidence-based efforts to improve patient care by empowering nurses with evidence-informed resources and training. The Resource Guide is now accessible through department SharePoint, supporting clinical conversations about medication adherence. Feedback and updates to resources ensure sustained relevance and efficacy, positioning nurses to better address patient barriers and contribute to improved health outcomes and performance metrics.
