Abstract
Background
The purpose of the “Pit Crew” in the progressive care unit's admission process is to improve patient care quality. Admissions are complex and lengthy, often missing critical assessments or interventions. The “Pit Crew” ensures key assessments and interventions are done simultaneously by utilizing a team approach, enhancing compliance with skin assessments, CHG baths, MRSA nasal swabs, and admission assessments. Additionally, it improves communication and collaboration among healthcare providers, boosts staff satisfaction and efficiency, and reduces burnout. This approach aims to deliver high-quality, patient-centered care while supporting the healthcare team in providing effective services.
Local Problem
The target audience includes bedside nurses, nurse educators, quality improvement nurses, and nurse leaders. This project enhances coordination, communication, and patient safety, benefiting both patient care and nurse satisfaction. Improved teamwork leads to better retention, communication, and patient satisfaction scores. Knowledge levels range from novice to expert, offering learning opportunities, confidence building, and leadership roles depending on the nurse's experience.
Method
Leadership used a PDSA cycle to create a team initiative for collaboration during patient admissions. Goals included improving identifying wounds on assessment, CHG baths, MRSA swabs, patient outcomes, and staff satisfaction. Effectiveness is measured by patient satisfaction, safety event reporting, documentation compliance, and outcomes. This initiative can be applied to other units for similar benefits.
Results/Conclusion
The project led to a 160% increase in safety event reporting from November 2024 to April 2025. It also resulted in a reduction of unit-acquired pressure injuries from 12 in Q4 2024 to 0 in Q1 2025, an increase in patient’s Likelihood to Recommend from 73.7% in November 2024 to 83.3% in March 2025, and improved documentation compliance of unit quality metrics.
Implications for Practice
For nurses, this team-based approach offers mentorship and support for new nurses, enhancing their learning and confidence, and leadership opportunities for expert nurses to share their knowledge and experience.
